Registration information for all participants

 

For new participants

  • Create an account in our Audit tool.
  • Search for your service. If the details are already logged, link to that service and register it for the 2025 Audit.
  • If your service details are not logged in the tool, add the details and register it for the 2025 Audit.
  • You can register more than 1 service.

If you've taken part in the Audit before

  • Go to our Audit tool and log in to your account. If you need a reminder of your login details, contact [email protected].
  • If you'd entered your service details for previous Audits, register your existing service(s) for the 2025 Audit.
  • Add the details of a new service (or services) and register it for the 2025 Audit.

Essential information for all participants

  • Each specialty needs to register separately. For example, if you're part of a multidisciplinary team, you will need to register each eligible service within it.
  • Each service only needs to be registered once. Additional users will be able to create an account when data collection opens on Thursday 1 May to enter and review data for registered services.

Therapy services should register as occupational therapy, physiotherapy or speech and language therapy, not as elderly care or neurology. These are the patient management audits for consultants or nurse specialists.

What happens next?

You'll receive email confirmation of your service's registration with your service number and a handy Audit checklist. Make sure you print out or save the checklist.

Data collection opens on Thursday 1 May. You'll then be able to download printable patient case sheets from our Audit webpage for your Patient Audit data.

Look out for your Audit pack of Patient reported experience measure (PREM) materials which will be delivered, by post, in April. Pharmacy services won't receive a pack of PREM materials.

Case study: registering a group of therapy services for the Audit

If you're registering for the Audit as part of a multidisciplinary team, to help you get the most from the process, this fictitious case study shows you the steps that a group of therapy services would need to take to register correctly for the Audit.

We're a group of therapy services made up of physiotherapy, occupational therapy and speech and language therapy teams and we want to register for the 2025 Audit.

Because our service hasn't taken part in the Audit before, someone from the service needs to create an account in the Audit tool and then add and register each of the therapy services separately for the Audit.

Alternatively, we could choose to have a lead person from each therapy specialty create an account in the Audit tool and then add and register their individual service for the Audit.

Our overall service will have 3 Audit registrations: ‘occupational therapy’, ‘physiotherapy’ and 'speech and language therapy'. Even though we consider ourselves to be a 'neuro' service, we don't register for the Audit under ‘neurology’.

If our service had taken part in the Audit before, we would already have an account. We would add the existing services to the account and register them for the 2025 Audit.
